Health Waiver
For all company members, partners, and students.
Participants/Associates understand and agree that in participating in any activity and services including but not limited to classes, workshops, rehearsals, conferences, fundraisers, or performances, provided by and under the services of Soul Movement LLC, there is a possibility of physical injury or death. Participants/Associates voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of the dance classes, rehearsals, performances, or activities. Participants/Associates also exempt, release, and indemnify Soul Movement LLC, its owners, instructors, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Soul Movement LLC.
Participants/Associates further hereby voluntarily agree to waive their rights and that of their heirs and resign to hold Soul Movement LLC, its owners, instructors, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. Participants/Associates understand that they should be aware of their holistic limitations and agree not to exceed them.
Use of Image
For all company members, partners, and students.
Participants/Associates understand and agree that in participating in any activity and services including but not limited to classes, workshops, rehearsals, conferences, fundraisers, or performances, provided by and under the services of Soul Movement LLC and respective dbas, there is a possibility of recorded photography and videography. By participating in services and activities of Soul Movement LLC, consent to use their image, likeness, and/or voice in photographs, video recordings, or other media captured or produced is assumed at the time of receiving service, for the purpose of marketing and promotional materials, including but not limited to print, digital, and social media platforms. Participants/Associates voluntarily agree, therefore, to the following terms:

Soul Movement LLC has full rights to use, reproduce, and distribute the aforementioned media content in perpetuity, without any further compensation to me.

Participants/Associates waive any right to inspect or approve the finished product wherein my likeness appears. This includes written or electronic copy associated with the media content.

Participants/Associates release, discharge, and hold harmless Soul Movement LLC and its representatives from any and all claims, demands, or causes of action that they, their heirs, representatives, or assigns may have by reason of this authorization.

Participants/Associates understand that once their image is published or distributed by the Organization, it may be viewed by the general public. They agree that Soul Movement LLC is not responsible for any misuse of the media content by third parties.

Any signed consent form shall be binding upon their heirs, legal representatives, and assigns.
